Here's what I found from googling it:

After setting voice_enable to 0, if you have done this, it will mute all mics from other players on the server (client-side, of course). When you set voice_enable back to 1, it cannot assign a correct codec channel for a certain microphone or person (see clients (19, 13, etc)). Try going into the Options menu, and hitting apply; this has been proven to work in some cases.

For your case, since it crashed, and it said:

Netchannel: failed reading message svc_VoiceData from 64.34.210.98:27015.

This problem is usually because of the server itself, or because you have to set your rate to a different value. On your side, you should try setting your rate lower than it is now; in one example, someone changed their rate from 25000 to 9999, and it has proven to fix the issue. If not, we'll have to contact Haggard or a BD to look at the server and see if anything is wrong with it that would cause this issue- but I'm pretty sure it's on your side of the connection, not ours.

Try out these steps and see what happens. If not, I can research more; for now, though, this is all I came up with. Good luck!
